We have 15 population health metrics in four areas: Healthy Babies, Healthy Children, Healthy Bodies and Healthy Minds. More details here. Initiative website: http://healthiersc.org/ Get Connected: Facebook Twitter Collective Impact Elements in Place Common Agenda Continuous Communication Shared Measurement Backbone Support Mutually Reinforcing Activities Backbone Support Organizations serving in the Backbone Role Catalyst for Health Department at the South Carolina Hospital Association Backbone structure One of the 55 member organizations was selected to provide the backbone support. The Alliance members contribute an annual membership fee to support staff and activities. Measures of Success See metrics here. Infant Mortality Disparities in Low Birth-weight Reading at grade level in third grade (and economic disparity) Racial Disparity in ED utilization Preventable Hospitalizations (and behavioral health disparity) Chronic disease management outcomes (asthma, diabetes, hypertension) Well-child visits Proportion of people who needed to see a doctor but couldn't due to cost Proportion of low-income people who spent more than a week last month feeling mentally unhealthy Initiative Activities and Milestones Activities Championing the Call to Action for Health Equity in South Carolina Raising awareness about existing collaborative health improvement plans for each priority area; as well as champion front-line/expert coalitions in each area Supporting the implemention fo the existing collaborative plans Consensus building around controversial policy issues Raising support for the incorporation of the Common Agenda in the strategic plans of the member organizations and partners Milestones 100% growth in membership in the last 3 years. 80% participation of members at regular meetings. Two of the 15 goals for 2020 have already been achieved. (Success is not directly attributable to the Alliance, but to the front-line/expert coalitions that work on the priority areas of the Alliance) Launching of a hub website that directs people to the different state-level coalitions working on health outcomes. Coalitions that existed prior to Alliance launching are now choosing to connect with the Alliance for support on policy and systems level changes Community Engagement and Advancing Equity Community Engagement Activities Members are encouraged to incorporate community advisory groups in all their initiatives as part of the Call to Action for Health Equity. Two new funding opportunities created by Alliance members are incorporating people with lived experience in the design of the local initiatives. Consumer groups that are part of the Health Equity team offer support on grassroots community engagement. Activities to Advance Equity All priorities have an overall goal, and an equity goal. The Alliance has launched a call to action for health equity with four components. Technical assistance is provided for organizations interested in implementing it. 1. Stratification of data by race, ethnicity, language, income, and zip-code 2. Cultural humility assessment and training 3. Inclusive decision making (from pipeline to recruitment) 4. Community Engagement Initiative Partners and Governance Contributing organizations 55 organizations including: Businesses, Consumer Advocates, Healthcare Providers, Health Insurance Companies, Government Agencies, Funders, Policy Analysts, and Universities. Sectors Engaged Nonprofit Philanthropic Funders Public Sector Business Sector Initiative governance (See details here) The Alliance is a group of 55 organizations. The CEO/Executive Director/Board Chair is the main representative of the organization in the Alliance, and they all meet once per quarter. A subset of this group is elected to be part of the Leadership Team (3 year terms). They meet right after Alliance meetings. This team champions strategic, systems/policy consensus building and action. A 9-member Operations Team coordinates implementation of Alliance recommendations in between meetings, coordinates the work of all Alliance Teams, and serves as the main connector with the front-line coalitions working on priority areas of the Alliance. This team is currently in the process of developing a strategy to incorporate community activation. Communications, Health Equity, Policy and Advocacy, and Development are teams open to any staff of member organizations. Discussions
